This role is based within Community Health and Therapy Services (CHATS) , which is crucial for consumer care. You will be instrumental in assessing functional capacity, implementing re-enablement strategies, and prescribing complex equipment/home modifications to ensure consumers successfully transition from acute care back into their community. Must demonstrate the ability to apply advanced clinical skills across a range of complex conditions.
